Use this form to request enrollment of a participant's phone number in Beaufort AI's wellness companionship SMS service (the "Companion"). This form is completed by the family member, legal guardian, or authorized caregiver enrolling the participant — not the participant themselves.
Submitting this form does not immediately enroll the number. A Beaufort AI representative will contact the enrolling party to verify identity and authorization before activating the Companion service. The participant will receive a confirmation opt-in text message before any other messages are sent, and the participant may independently reply STOP at any time to end messages, regardless of who requested enrollment.
Declining SMS consent above does not prevent you from submitting this request or contacting Beaufort AI for program information.
